In collaboration with the Pharmaceutical Society of Australia and the Western Australia Primary Health Alliance, The Change Hub led the implementation of the updated heart failure guidelines. Participants included general practitioners, pharmacists, nurses, and other healthcare professionals.
AIM:
The aim of the project was to provide the necessary tools and knowledge for healthcare professionals to increase inter-disciplinary collaboration and implement services to help their patients manage their heart failure.

General practice clinics and pharmacies created new heart failure services that incorporated feedback from their entire team as well as from patients.
