Below are examples of facilitated change projects with our clients

WHO:

In collaboration with the Pharmaceutical Society of Australia and the Western Australia Primary Health Alliance, The Change Hub led the implementation of the updated heart failure guidelines. Participants included general practitioners, pharmacists, nurses, and other healthcare professionals.


AIM:

The aim of the project was to provide the necessary tools and knowledge for healthcare professionals to increase inter-disciplinary collaboration and implement services to help their patients manage their heart failure.


WHAT WE DID:

  • Co-designed a change implementation guide tailored to heart failure.
  • Trained different healthcare professionals (general practitioners, pharmacists, nurses) in Western Australia on ways to navigate change in their practice.
  • Provided in-house facilitation support to help teams design a service that is tailored to their patient and practice needs.
  • Conducted follow-up calls and webinars to ensure continuity and sustainability of the service.


OUTCOME:

General practice clinics and pharmacies created new heart failure services that incorporated feedback from their entire team as well as from patients.


WHO:

The Digital Health Agency and the Pharmaceutical Society of Australia.


AIM:

To improve the adoption of My Health Record (MHR) by Pharmacists.


WHAT WE DID:

  • Explored pharmacist challenges and concerns regarding MHR.
  • Co-designed the pharmacy implementation guidelines for My Health Record.


OUTCOME:

Increased use of MHR by pharmacists nationwide.


WHO:

Antares Solutions - a software development company


AIM:

To develop an adoption framework that can be used during the deployment of their software products to ensure end-user adoption


WHAT WE DID:

  • We conducted change management training with their project managers, business analysts, change managers, software developers, and product owners to create alignment around adoption.
  • Co-designed a change framework including collateral relating to communication, training, support, case studies, and more.
  • Provided ongoing change management consulting to ensure the adoption framework is embedded in every implementation project.

OUTCOME:

  • The entire team is using the same language, everyone understands why adoption is important and how to create synergy between adoption and project management.
  • Clients have felt more supported due to the in-depth adoption approach and are more willing to use their products.
